We have an exciting opportunity for a full-time Executive Chef to join The Vista, an independent adult resort style living community. The Executive Chef, reporting to the Dining Services Director, will lead the culinary services team in the Vista facility to ensure compliance with regulatory standard while working towards improving systems and processes. The Unit Secretary will be responsible for the clerical functions in the unit and acts as a receptionist. Christian Health offers a unique opportunity for student nurses to learn valuable skills and gain the confidence necessary to be successful in their nursing careers. The Christian Health Student Nurse Program (SNP) provides an environment that is collaborative and conducive to learning which benefits the patient, the health-care facility, and the nursing profession.
Student Nurse Program (SNP) participants are educated in psychiatric mental health adult and geriatric nursing; which includes nursing assessment and interventions; interdisciplinary care planning; psychiatric and mental-health nursing; and critical-thinking skills. Student nurses participate in classroom orientation before being assigned to clinical practice areas. Once in the clinical area, student nurses will continue to receive education from their work experience, attendance at in-services, and participation in a lecture series, designed specifically for SNP participants. Educational topics include therapeutic communication, suicide risk and interventions, managing medical conditions, fall prevention, individualized interventions/reduction in targeted behaviors and evidence based care for patients with dementia.
Under the supervision of a Registered Nurse, student nurses provide direct patient care to their assigned patients. Student nurses assume responsibility and accountability for patient care as a team member, implement interventions, and assist patients in achieving positive outcomes. Through this program, student nurses, who are paid, have the opportunity to apply newly acquired knowledge and skills in the day-to-day practice of nursing.
Responsibilities include:
- Performing data collection and documentation of vital signs, weight, and intake and output.
- Performing EKGs and blood-glucose checks.
- Providing assistance with activities of daily living i.e. hygienic care, ambulation, meals, and nutrition.
- Ensuring a safe patient environment by observing the patient and environment for any potential or actual safety concerns, i.e. answering call lights promptly and responding to fall risk.
- Observing for any changes in behavior, mood, or ability.
- Collaborating with the registered nurse on any needed updates to the nursing care/treatment plan relative to observed and reported information regarding patient status or condition.
- Providing relevant patient/family education under the supervision of the Registered Nurse.
Scheduling requirements:
- During the academic year, SNP participants will be offered a flexible schedule.
- During the summer, SNP participants will be required to work a minimum of three days per week with an every-other-weekend rotation.
- Hours: Must be flexible to rotate between 7am-3pm and 3pm-11pm shifts.

We have an exciting opportunity for a full-time Admissions Liaison will interface with referral sources to assist in the coordination and facilitation of admissions/readmission to Bolger Short-term rehab, Heritage Manor Nursing Home and Southgate Special Care through clinical review of potential clients. Will work closely with the Admissions Team to ensure a seamless referral-to-admission process while building a strong referral base. Will work collaboratively to foster excellent relationships with key partnerships with an emphasis on marketing and consumer awareness of Campus programs. Schedule: Monday – Friday 8:30am-4:30pm.
Education/Experience: Minimum of 2 years related experience, preferably in clinical case management; knowledge of Medicare PDPM, MDS in LTC setting; marketing and program development. Certified Case Manager (CCM) preferred.
Competencies:
- Manages the facility census by acting as a liaison between external referral sources and internal operations.
- Demonstrates growth in hospital referrals, as reflected in an increase of overall program occupancy.
- Identifies and cultivates ongoing relationships with viable referral sources (including but not limited to: hospital personnel, physicians, discharge planners, social workers, nurse case managers, rehab staff, management staff, alternate care facilities, geriatric care managers, insurance representatives, clients and consumers, etc..
- Screens patients to establish eligibility for care at CH by review of clinical charts/EMR and on-site personal visits when indicated. Makes house calls when necessary.
- Obtains accurate information, verifies payer source, goals of care, and discharge plan.
- Interfaces with physicians to verify anticipated care and treatment directives.
- Communicates with Admissions Director/DON/Administrator or designee regarding census-driven needs, including room changes, inter-campus transfers, readmissions, etc.
- Possesses clinical savvy to identify and intervene concerning hospital discharges in an effort to avoid unnecessary re-hospitalizations.
- Works collaboratively with Admissions Team members as well as nursing and other internal and external staff to facilitate referral conversion through skilled communication and positive motivation. Decides acceptance of patients and coordinates admission process with facility staff.
- Demonstrates fiscal responsibility and accountability by performing pre-admission cost-analysis to determine cost of patient care (i.e. medications, treatments, equipment, etc.) vs. reimbursement. Confers with Supervisor/Administrator to obtain consent to admit outlier cases.
- Ensures PASRR collection/compliance prior to admission.
- Tracks referrals and follows hospitalized patients to facilitate readmissions to the facility (when appropriate).
- Enhances program visibility by participates in various external marketing events and community partnership opportunities.
- Supports ACO partnerships by attending collaborative meetings, fostering good communication between organizations and disseminating information to appropriate facility personnel.
- Refers patient/resident representative to Admission Team for completion of tours, admission paperwork and financial review.
- Identifies and assists in coordinating the acquisition of any needed specialty equipment, supplies, medications and/or consultations prior to admission.
- Documents and distributes appropriate and complete information to the Care Team via current process.

The Psychiatrist will provide professional time to the Ramapo Ridge inpatient unit maintaining the quality of care in a Christian and professional manner to all patients assigned to him/her. Functions as Psychiatrist and provides needed therapeutic intervention, integrating Biblical principals into treatment modalities, as appropriate to patient's needs. The Psychiatrist will need to be competent to care for patients in the following age groups: adolescent (ages 14 - 18), adult (ages 19 - 64) and geriatric (ages 65 +). - Perform timely charting and documentation of all care provided, in compliance with all applicable regulations and standards.
- Screens and admits patients to hospital.
- Provides Psychotherapy to assigned patients as needed.
- Attends Medical staff meetings as required.
- Dictates discharge summaries in a timely manner.
- Provides coverage as assigned.
- Serves as on call Psychiatrist for the hospital, at night and on weekends as assigned.
- Provides supervision to clinical staff as assigned/is necessary.
- Provides patient care services within the scope of clinical privileges granted by the governing body.
Schedule: Full-time, Exempt position, On-call rotation required.
Education:
- Graduate of a Medical School approved by the Council on Medical Education and Hospitals of the American Medical Association and successful completion of Residency Training in General Psychiatry.
- Licensed to practice medicine in the State of New Jersey.
