The Field Nurse Home Care RN conducts home visits to assess clients and provide community outreach. Works closely with intake staff to obtain assignments of new cases and schedule client visits. Will be responsible for the supervision/evaluation of plan of care provided to the Certified Home Health Aides (CHHA's) in the client's home.    Schedule: Full-time Monday - Friday 8am - 4pm. Evening and weekend hours as required.   Education: RN degree with valid RN licensure in New Jersey required.

Surround yourself with people who care about making a difference – come join us!   We have an exciting opportunity for a Full-time, Psychiatric Nursing Assistant (PNA) to join Ramapo Ridge.  Under the direction of the RN, the Psychiatric Nursing Assistant (PNA) performs the activities related to the implementation of nursing care plans and the nursing standards, policies and procedures. The PNA is responsible for contributing to the development, evaluation and reassessment of care plans through observing and reporting changes in patient behavior and physical status. The PNA is also responsible for assisting in the maintenance of the safe and therapeutic environment of the unit. The individual is competent to care for patients in the following age groups: adult (ages 19 - 64) and geriatric (ages 65 +). Demonstrates an understanding of, and embraces, the mission statement of the Christian Health Care Center.   Competencies: - Supervises or assists in after meal care (removing trays, encouraging patients in brushing teeth, washing up, etc.). - Serves food trays. Assists with feeding as indicated.  - Assists patients in preparing for activity and social programs, as per unit program schedules. - Answers call bells promptly. - Changes bed linens. - Assists or supervises patients to walk with or without self-help devices as instructed. - Reports all changes in the patient's condition to the Charge/Staff Nurse as soon as practical. - Keeps floors dry. Verifies all insurance coverage eligibility, benefit details, pre certification/authorization requirements and billing/claims instructions. Ensures accurate insurance data is available in the medical record.  Works collaboratively with the interdisciplinary treatment team to review all managed care medical record documentation for compliance as required in a timely manner. Ensures that medical record documentation for patients admitted to RRBH meets medical necessity criteria for each level of care. Provides for timely and accurate clinical reviews as required to ensure compliance with managed care organizations. Follows up and assesses any managed care denials for risk/benefit of appeal. Functions as Psychiatrist and provides needed therapeutic intervention, integrating Biblical principals into treatment modalities, as appropriate to patient's needs.
