We have an exciting opportunity for a Full Time Mental Health Counselor to provide compassionate mental health support through Christian Health’s Home-Based Counseling Program for Older Adults. Brings professional counseling services directly to the homes of Bergen County adults age 60 and older and their caregivers, especially those who are homebound, isolated, or face barriers to accessing outpatient care. Provides assessment, reassessment, individual and family counseling, psycho-education, care coordination, and connection to community resources that support emotional well-being, improve daily functioning, and help older adults remain safely in their communities. Maintains clinical documentation, participates in supervision and treatment team processes, and demonstrates an understanding of and commitment to the mission of Christian Health.

 

Competencies:

  • The individual is competent to provide home-based counseling services to older adults age 60 and older, with sensitivity to the needs of frail, isolated, low-income, and other underserved individuals. 
  • Assists in providing appropriate planning and preparation for referrals, assessments, and coordination of care for older adults and caregivers receiving home-based services. 
  • Coordinates care and consults with caregivers, family members, and community partners to support integrated, goal-oriented services for older adults in their homes. 
  • Provides supportive in-home counseling services, including individual and family counseling, psycho-education, case management, and social skills support, to help older adults regain emotional well-being and improve daily functioning.
  • Provides accurate and timely documentation of counseling services and related client information in required records and reporting systems. 
  • Completes assessments and reassessments of mental health counseling needs, providing timely clinical summaries and initial/interim treatment plans as assigned. 
  • Remains current in program standards, regulations, counseling practices, and community resources relevant to serving older adults and caregivers in a home-based setting. 
  • Attends and participates in regularly scheduled clinical, administrative, supervisory, and treatment team meetings as required. 
  • Prepares for and participates in monthly County treatment team meetings and other grant-related program meetings as needed. 
  • Meets with older adults and caregivers during the assessment process as appropriate, incorporating caregiver and family input into service planning when relevant. 
  • Assists in formulating and facilitating counseling and psycho-educational services for older adults and caregivers in ways that reduce barriers to care and increase access to support.

 

Schedule: Full-time, Monday - Friday 9am-5pm.

 

Education: Masters Degree from an accredited graduate program in psychology, social work, counseling or marriage and family counseling.

 

Special Skills/License: Licensed Social Worker (LSW) required or LSCW.

 

Equipment Utilization: Use of own automobile to visit consumers in their homes throughout Bergen County. Use of Mon Ami software system for statistical record keeping.

QUALIFICATIONS/REQUIREMENTS

Demonstrates compliance with the CH ASPIRE Standards of Performance.

 

Qualifications:

  • Minimum of one (1) year counseling experience in a mental health setting, volunteering, or internship.
  • Licensed Social Worker (LSW) required or LSCW.
  • Excellent communication skills are required to effectively handle referrals and consultations with various other staff members, including establishing and maintaining excellent relationships with referral sources and other Bergen County organizations.
