We have an exciting opportunity for a full-time External Case Manager-Liaison in Heritage Manor/Southgate Admissions.  The External Case Manager will interface with referral sources to assist in the coordination and facilitation of admissions/readmission to Bolger Short-term rehab, Heritage Manor Nursing Home and Southgate Special Care through clinical review of potential clients. Will work closely with the Admissions Team to ensure a seamless referral-to-admission process while building a strong referral base. Will work collaboratively to foster excellent relationships with key partnerships with an emphasis on marketing and consumer awareness of Campus programs. Demonstrates an understanding of and embraces the mission and vision of Christian Health.

 

Competencies:

  • Enhances program visibility by participating in various external marketing events and community partnership opportunities; markets the facility programs and services to health-related audiences and consumers.
  • Manages the facility census by acting as a liaison between external referral sources and internal operations.
  • Utilizes strong organizational skills, coordinates activities and communications among people, agencies and organizations to build and maintain mutually beneficial relationships with a goal of generating repeat referrals.
  • Ensures knowledge of campus resources; refers to alternate programs as applicable.
  • Maintains knowledge of and collaborative relationships with competitor facilities.
  • Demonstrates growth in hospital referrals, as reflected in an increase of overall program occupancy.
  • Identifies and cultivates ongoing relationships with viable referral sources (including but not limited to: hospital personnel, physicians, discharge planners, social workers, nurse case managers, rehab staff, management staff, alternate care facilities, geriatric care managers, insurance representatives, clients and consumers, etc..
  • Screens referrals to establish eligibility for care at Christian Health by review of clinical charts/electronic medical records and conducting on-site personal visits when indicated. Interfaces with physicians to verify anticipated care and treatment directives. 
  • Obtains accurate information, verifies payer source, goals of care, and discharge plan.
  • Conducts Pre-admission/readmission assessment visits providing information for completion of initial nursing assessment in accordance with regulatory guidelines.
  • Works closely with Nursing, Clinical Education, Dietary, Rehab departments to ensure adequate capabilities prior to acceptance.  
  • Communicates with Admissions Director/DON/Administrator or designee regarding census-driven needs, including room changes, inter-campus transfers, readmissions, etc.
  • Possesses clinical savvy to identify and intervene concerning hospital discharges in an effort to avoid unnecessary re-hospitalizations.
  • Works collaboratively with Admissions Team members as well as nursing, medical and other internal and external staff to facilitate referral conversion through skilled communication and positive motivation. Decides acceptance of patients and coordinates admission process with facility staff.
  • Demonstrates fiscal responsibility and accountability by performing pre-admission cost-analysis to determine cost of patient care (i.e. medications, treatments, equipment, etc.) vs. reimbursement.
  • Confers with Supervisor/Administrator to obtain consent to admit outlier cases.
  • Interfaces with prospective client’s family/responsible party to offer introduction, information, program overview, support, assistance with admission-related matters.
  • Refers patient/resident representative to Admission Team for completion of tours, admission paperwork and financial review.
  • Ensures PASRR collection/compliance prior to admission. Works effectively with the Office of Community Choice Options (OCCO).
  • Tracks referrals and follows hospitalized patients to facilitate appropriate readmissions to the facility.
  • Supports ACO partnerships by attending collaborative meetings, fostering good communication between organizations and disseminating information to appropriate facility personnel.
  • Provides feedback and recommendations for program development, quality and clinical improvement initiatives; participates in activities related to clinical outcome data collection and performance improvement.
  • Promotes patient satisfaction goals, supports PFCC initiatives and conscientiously works to raise/sustain favorable satisfaction scores.
  • Identifies and assists in coordinating the acquisition of any needed specialty equipment, supplies, medications and/or consultations prior to admission.
  • Documents and distributes appropriate and complete information to the Care Team via current process.
  • Demonstrates awareness of legal issues in aspects of resident care and facility function and strives to manage situations in a manner that reduces risk.
  • Assists with the development of annual goals, objectives and strategic plans and maintains focus on departmental and organizational goals.
  • Maintains positive, optimistic attitude and outlooks and displays willingness to perform other duties as required.
  • Provides coverage for peers on weekends as needed.

Schedule: 9am-5pm, Monday - Friday, weekend coverage as needed.

 

Education: Bachelor’s Degree in related field required (i.e., nursing, social work, etc.). Certified Case Manager preferred.

QUALIFICATIONS/REQUIREMENTS

Demonstrates compliance with the CH ASPIRE Standards of Performance.

 

Qualifications:

  • Minimum of 3-5 years related experience in clinical case management in the areas of long term care, mental health/psychiatric population, and dementia care.

  • Knowledge of Medicare PDPM, MDS in LTC setting; marketing and program development and excellent interpersonal, organizational and communication skills.

  • Knowledge of LTC regulations such as MDS, Medicare, Medicaid, ACO, PASRR, OCCO, SCNF, Managed & Commercial insurance a plus.

  • Energetic, strong interpersonal skills and ability to multitask.

  • Forge strong relationships with physicians, hospital nursing/case management/social services, floor & discharge staff.

  • Strong clinical background & medical assessment skills.

  • Ability to make sound clinical decisions to ensure appropriate placement in SNF & SCNF.

  • Knowledge of computer software including Word and Excel. PAS, PASRR, PASRR Level II, and OCCO requirement savvy.

  • Travel required.
