We have an exciting opportunity for a full-time, Social Worker to join our Gracepoint partial hospitalization program. The Social Worker provides clinical and concrete social work services in the setting of a mental health partial hospitalization program including individual, group and family psychotherapeutic, supportive and educational counseling. The individual is competent to care for patients in the following age groups: adult (ages 18 - 64), and geriatric (ages 65 +). Demonstrates an understanding of, and embraces, the mission statement of Christian Health.

 

Competencies:

  • Conducts psychotherapeutic counseling and psychoeducation to consumers with a primary mental health diagnosis including those in recovery from a co-occurring substance use disorder. 
  • Provides individual and family counseling. 
  • Assists in maintaining accurate, legible treatment documentation, as per PHP policy, regulation, licensing and accreditation requirements. 
  • Interviews patient, family, and significant others to obtain complete psychosocial history to complete Clinical assessments in a timely and accurate manner. 
  • Provides, with appropriate patient consent, support, education and assistance to families regarding patient's diagnosis treatment, and discharge, including information and referrals required for appropriate discharge, i.e.: Medicaid/Medicare.  
  • Seeks and utilizes resources of community agencies, referring patients to available, appropriate community resources and services. Facilitates communication with collateral providers from admission through discharge and follow-up.  
  • Conducts patient and family interviews/sessions, as well as individual and group therapy, education and supportive services and may function as primary therapist in identified cases. 
  • Facilitates continuity of care through timely and complete communication with collateral providers. 
  • Assesses discharge planning needs to facilitate implementation of effective Discharge plan in a timely & accurate manner. 
  • Assist in screening possible Admissions together with PHP Interdisciplinary team. 
  • Serves as a social work and clinical resource to PHP interdisciplinary team. 
  • Attends interdisciplinary treatment team meetings. Participate in the development of treatment plans. 
  • Contributes to the PHP Utilization Review process. 
  • Utilizes behavior measurement scales. 
  • Participates in the program's Quality Improvement Activities. 
 

Experience: At least two years experience in a hospital or psychiatric outpatient clinic preferred.

 

Schedule: 11am-7pm, Monday - Friday, weekend coverage when needed.

 

Education: MSW from an Accredited School of Social Work, LSW required; LCSW preferred.

 

Licensure: Current valid New Jersey Licensed Social Work (LSW) required.

QUALIFICATIONS/REQUIREMENTS

Demonstrates compliance with the CH ASPIRE Standards of Performance.

 

Qualifications:

  • At least two years experience in a hospital or psychiatric outpatient clinic preferred.
  • Must be licensed by the State of New Jersey LSW or LCSW.
  • Must be proficient in Microsoft Office including Word and Excel.
