We have an exciting opportunity for a full-time, Social Worker to join our Gracepoint partial hospitalization program. The Social Worker provides services to psychiatric patients and their families including individual, group and family supportive and educational counseling, care coordination and post-acute care planning. The individual is competent to care for patients in the following age groups: adult (ages 18 - 64), and geriatric (ages 65 +). Demonstrates an understanding of, and embraces, the mission statement of Christian Health.
Competencies:
- Conducts individual, group and family psychotherapeutic counseling and may function as primary therapist in identified cases.
- Completes psychosocial assessments including obtaining information for diagnosis and treatment as required by regulatory and accrediting agency requirements. Personality characteristics and interrelationship perceptions as indicated by the patient and by the family should be noted.
- Serves as liaison to patients and families and available community partners and resources.
- Maintains accurate, timely documentation as a basis for review by the treatment team and accrediting agencies.
- Provides support, education and assistance to families and their support system regarding community resources, course of treatment and hospitalization, and discharge planning.
- Provides information and referrals required for appropriate placement i.e. Medicaid-Medicare.
- Collaborates with public defender's office and court adjuster’s office regarding civil commitment hearings for involuntary patients.
- Coordinates and facilitates post-acute care plan for assigned patients.
- Participates in Department QI activities.
- The individual is competent to care for patients in the following age groups:- adult (ages 18 - 64), geriatric (ages 65+).
- Explains patient Bill of Rights to patient's legal guardian and/or POA.
- Engages in rounding and service recovery.
- Attends and actively participates in Interdisciplinary Treatment Team Meetings, prepared to offer psychosocial assessment information to guide treatment. Participate in the development of wellness and recovery plans.
- Provides coverage for department colleagues.
- Provides coverage for Utilization Review team as needed.
- Completes continued stay authorizations/concurrent reviews as assigned. And documents/communicates the results of reviews with all interdisciplinary treatment team members.
- Demonstrates competency in the utilization of restraints, via post test and return demonstration.
- Supervises Bachelor and Master's level student interns, when applicable.
- Assists in developing favorable attitudes in the community toward recognition and treatment of mental illness and the aging process through participation in community activities concerned with mental health and/or aging.
- Performs other related duties as required.
Schedule: Full time, 8am-4pm Monday - Friday.
Education: Master's degree in Social Work required.
Licensure: Current valid New Jersey Licensed Social Work (LSW) required.
Experience: At least one year experience in a hospital or psychiatric clinic setting preferred.
