We have an exciting opportunity for a full-time Home Care Nurse RN to join Christian Health's Home Care Options (HCO) department. The Field Nurse Home Care RN conducts home visits to assess clients and provide community outreach. Works closely with intake staff to obtain assignments of new cases and schedule client visits. Will be responsible for the supervision/evaluation of plan of care provided to the Certified Home Health Aides (CHHA's) in the client's home. 

Schedule: Full-time. Evening and weekend hours as required.

Education: RN degree with valid RN licensure in New Jersey required. 

• Minimum of one year of home care experience required.
• Must be willing to work in the field visiting clients in their home.
• Must have own transportation.
